Tripod Lite is a free to use incident investigation tool, designed for investigating simple incidents and near misses. With minimal training, it can be used by those expected to carry out minor investigations, including supervisors, managers as well as fully trained accident investigators. This online training course will give new users the grounding they need to use Tripod Lite to investigate simple incidents and near misses. Consisting of videos, quizzes, case studies and information sheets, learners will be introduced to the Tripod Lite tool, the basic theory behind it, and then be taken step-by-step through the Tripod Lite methodology.
During this course, learners will:
- Gain an understanding of the Tripod Lite tool, including:
- When to use Tripod Lite
- Its strengths and weaknesses
- How to navigate Tripod Lite
- How to fill out a Tripod Lite report
- Gain an understanding of the Tripod theory of incident causation and how this is represented in a Tripod Lite analysis.
- Learn how to model and analyse simple incidents and near misses using Tripod Lite, including:
- How to represent the timeline of an incident (‘what happened’) in Tripod Lite.
- How to identify the risk controls (‘barriers’) that fail and lead to incidents.
- Understand why barriers fail, and identify the immediate causes – including human actions – that lead to barriers failing.
- How to identify the human factors (‘job factors’, ‘performance influencing factors’) that influence people’s actions.
- How to identify organisational factors, including management system challenges, that help set incidents in motion.
This course is suitable for anyone who needs to use the Tripod Lite investigation tool, as well as those who want to gain an introduction to incident investigation and analysis.
